Open a new image size 500 by 500. 

C/P Element 33. Image, Rotate, Free Rotate 90 degrees. Resize by 45%.

Select the inside of the frame with your magic wand. Selections, Modify, Expand by 3.

Open Paper 5. Go to Image, Rotate, Rotate Clockwise. 

C/P Paper 5 as a new layer. Position as desired under the frame. Selections, Invert, Delete. Deselect. Dropshadow frame.

Duplicate the paper layer. Image, Rotate, Free Rotate 15 Degrees. Position under the original paper layer.

Duplicate frame layer.Image, Rotate, Free Rotate 15 Degrees. Position under the original frame layer.

Merge down the frame to the paper. Duplicate & Mirror.

Select the top paper layer. C/P your tube. Dropshadow. Use your selection tool to select around the frame. Selections, Invert, Delete. Deselect.

Select the top frame layer.

C/P Element 44. Resize by  50%. Dropshadow. Position about 1/3 of the way up the right side of the frame.

C/P Element 39. Resize by 30%. Dropshadow. Position over the bottom part of the heart on the tree.

C/P Element 31. Image, Mirror. Resize by 30%. Dropshadow. Position mostly over the top of the cage.

C/P Element 41. Resize by 50%. Dropshadow. Position the swirl over the bottom right corner.

C/P Element 37. Resize by 50%. Use your deform tool to position over the bottom right corner of the original frame. Dropshadow.

C/P Element 20. Image, Flip. Image, Mirror. Resize by 30%. Dropshadow. Position to the right of the floral branch.

C/P Element 22. Resize by 30%. Dropshadow. Position next to one of the purple flowers on the vine.

C/P Element 51. Resize by 10%. Dropshadow. Duplicate and mirror. Position over the angled frame on the left side of the tag.

C/P Element 32. Resize by 20%. Dropshadow. Duplicate. Position at the bottom of each of the hearts.

C/P Element 25. Resize by 30%. Dropshadow. Position at the bottom of the green flowers. 

Open Element 42. Using your lasso tool select the bird on the right. C/P as a new layer. Resize by 30%. Dropshadow. Position between the frog and fairy at the bottom of the frame.

Type out your name. Apply inner bevel of choice, Gradient Glow of 3, & Dropshadow.

Select your background layer. C/P Element 35. Position near the top of the frame. Dropshadow. Duplicate & Mirror.
